A) a site description (brochures, etc.),
B) names, titles, addresses and phone numbers of all supervisors,
C) a description of the anticipated internship activities including the types of cases to be seen, testing, consultation, in-service, etc.,
D) how the above activities will be evaluated by the site-supervisor.

Please note that the site-supervisor should provide at least 2 hours of direct supervision for every 40 hours of internship experience.
4. IUP has a formal written agreement
between the university and the internship site. This agreement has to be signed
by a signing authority for the internship site and approved by IUP. In
addition, the faculty supervisor will be corresponding by email to the site
supervisor regarding expectations of the internship experience. Please keep in
mind that both of these items need to be completed before you can start.
6. The student will also be required to submit sample reports for each 100 hour of internship experiences. These samples should reflect the wide range of cases you have experienced. These reports can be submitted along with the evaluation form that should be submitted after each 100 hours of experience. More details for these sample reports are also provided in the letter that is sent out to your supervisor.
8. It will be your responsibility to ensure that you have your own liability insurance - the internship site may require proof of this
8. At the end of your internship experience, submit all the above items (e.g., logs and sample reports) on a CD – I may post some of them on my Web page.
9. It will be your responsibility to contact me and request a “change of grade” for your internship. Please ensure that all the above items are completed before the grade change is requested.
